{"id":10179,"date":"2024-03-15T19:57:24","date_gmt":"2024-03-15T19:57:24","guid":{"rendered":"https:\/\/generic.wordpress.soton.ac.uk\/uosm2031-2024\/?p=10179"},"modified":"2024-03-15T19:58:31","modified_gmt":"2024-03-15T19:58:31","slug":"whats-new-in-diabetes-management-and-where-have-we-come-from","status":"publish","type":"post","link":"https:\/\/generic.wordpress.soton.ac.uk\/uosm2031-2024\/2024\/03\/15\/whats-new-in-diabetes-management-and-where-have-we-come-from\/","title":{"rendered":"What&#8217;s new in diabetes management and where have we come from?"},"content":{"rendered":"\n<p>I was contemplating David Simpson\u2019s lectures on sensors within prosthetics and was drawn to an <a href=\"https:\/\/www.todaysmedicaldevelopments.com\/article\/altek-artificial-pancreas-manufacturing-desin-32818\" data-type=\"URL\" data-id=\"https:\/\/www.todaysmedicaldevelopments.com\/article\/altek-artificial-pancreas-manufacturing-desin-32818\">article<\/a> he attached outlining the artificial pancreas device for type 1 diabetics. The device, also called the closed loop system (CLS), wirelessly connects a continuous glucose monitor (CGM) to an insulin pump to stabilise blood sugar levels automatically. This is revolutionary for diabetics who struggle to manage their blood sugars manually, and reduces the likelihood of long-term complications such as diabetic neuropathy (nerve damage). My mother is a type 1 diabetic, so I decided to look further into this.<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading has-large-font-size\">A brief <a href=\"https:\/\/jamaicahospital.org\/newsletter\/the-history-of-diabetes-testing\/\" data-type=\"URL\" data-id=\"https:\/\/jamaicahospital.org\/newsletter\/the-history-of-diabetes-testing\/\">history<\/a> of diabetes testing<\/h2>\n\n\n\n<p>Back in 600BC, scientists noticed that ants were attracted to the urine of those with diabetes and one brave (or questionable) lad personally confirmed its distinctive sweet taste. Fast forward to 1841 and we have the first clinical test for diabetes, before the 1940s produced urine test strips to quantify these results.<\/p>\n\n\n\n<p class=\"has-text-align-left\">Today the glucometer, first developed in 1970, is still widely used. It relies on a fingertip pin prick to draw blood for the test strip, which is then inserted into the meter. The <a href=\"https:\/\/engineering.mit.edu\/engage\/ask-an-engineer\/how-do-glucometers-work\/#:~:text=%E2%80%9CWhen%20the%20strip%20is%20inserted,%2C%20the%20higher%20the%20number.%E2%80%9D\" data-type=\"URL\" data-id=\"https:\/\/engineering.mit.edu\/engage\/ask-an-engineer\/how-do-glucometers-work\/#:~:text=%E2%80%9CWhen%20the%20strip%20is%20inserted,%2C%20the%20higher%20the%20number.%E2%80%9D\">physics behind this<\/a> is actually very interesting \u2013 the glucose in the blood sample reacts with glucose oxidase on the strip, generating an electrical signal. There\u2019s still a fair bit of maths to go wrong here though, as I saw following one horrifying mistake that left my mother guzzling 375g of dissolved sugar to counter an overdose on her fast-acting insulin.<\/p>\n\n\n\n<p class=\"has-white-color has-text-color\"><\/p>\n\n\n\n<p class=\"has-white-color has-text-color\"><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading has-large-font-size\">The CLS \u2013 a lottery win?<\/h2>\n\n\n\n<p>Significantly more automated, the <a href=\"https:\/\/www.diabetes.org.uk\/guide-to-diabetes\/diabetes-technology\/closed-loop-systems#:~:text=%E2%80%9CA%20closed%20loop%20system%20has,in%20my%20blood%20sugar%20levels.\" data-type=\"URL\" data-id=\"https:\/\/www.diabetes.org.uk\/guide-to-diabetes\/diabetes-technology\/closed-loop-systems#:~:text=%E2%80%9CA%20closed%20loop%20system%20has,in%20my%20blood%20sugar%20levels.\">CLS<\/a> aims to stabilise sugars with minimal input by combining the CGM with an insulin pump. Whilst users still need to compute carbohydrate intake, the main job of the CLS is to prevent hypos overnight by delivering tiny doses of insulin to keep blood sugars level. Hypos occur when sugars drop below normal range and can be extremely dangerous if not resolved quickly. <\/p>\n\n\n\n<p>A game-changer for lots of diabetics, one user claims it has \u201ccut out about 90% of [their] low level dips into hypoglycaemia\u201d. The principal drawback appears to be in roll-out due to relative cost. However, scientists have recently created tiny implants containing <a href=\"https:\/\/www.webmd.com\/diabetes\/news\/20231211\/stem-cell-therapy-implant-shows-promise-for-type-1-diabetes\">stem cell-derived pancreatic progenitor cells<\/a> that can emulate a healthy pancreas and produce insulin. The hope is that the encapsulated cells are protected from the diabetic\u2019s immune system which is intent on destroying them. <\/p>\n\n\n\n<p>Researchers are highly optimistic, with some even claiming this may \u201cturn into a cure a soon as 2024\u201d. However, I noticed a <a href=\"https:\/\/www.nature.com\/articles\/s41587-023-02055-5\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41587-023-02055-5\">potential ethical issue<\/a>, as implementation of the implant is reportedly limited by shortage of donor stem cells.
